Your healthcare choices are personal. Completing a Health Care Power of Attorney form allows you to list your preferences for end-of-life care. This is important for everyone, no matter their age or health status. The Power of Attorney form makes it possible for adults to authorize other individuals to make health care decisions on your behalf if you are unable to do so yourself.
