Patient Bill of Rights
Fox Valley Hematology & Oncology, S.C. [FVHO] respects the rights of our patients. We recognize that each patient is an individual with unique healthcare needs. Due to the importance of valuing each patient’s personal dignity, we provide considerate, courteous care focused on the patient.
Fox Valley Hematology & Oncology, S.C. has three goals in creating a Patient Bill of Rights:
- To ensure patients feel confident in the care provided by FVHO.
- To emphasize the importance of the bond connecting the patient and their healthcare provider.
- To illustrate the key role that each individual plays in their own healthcare outcomes.
Our first priority is to provide you with the care that you need—when you need it—with compassionate, experienced providers.

A Clean and Safe Environment
Each patient has the right to a clean and safe treatment environment.
Access to Information
You have the right to access information concerning your treatment, laboratory results, insurance and billing information.
FVHO provides free aids and services to people with disabilities to communicate effectively with us, such as qualified sign language interpreters and written information in other formats (large print, audio, accessible electronic formats, other formats).
For patients whose primary language is not English, FVHO provides free language services, such as qualified interpreters and information written in other languages.
If you need these services, please contact a member of staff or your patient navigator.
Access to Urgent Care and Emergency Services
Patients can reach FVHO during our normal business hours (Monday through Friday, 8:30 AM until 5 PM) or after hours through our answering service. In case of emergency, patients are encouraged to contact 911.
Involvement in Your Care
Each patient has the right to know and discuss his or her medical condition and appropriate treatments.
At FVHO we strive to make sure patients understand their healthcare goals, values, and outcomes. Providers will discuss treatment options with patients and family members to ensure the right choice is made for treatment.
Involvement in your care means that at times, providers and their staff require information from you. To ensure the best possible outcomes, patients are requested to comply with such requests for information.
Respect and Non-Discrimination
You have the right to compassionate, courteous care from your physician, nurse practitioner and clinical staff at FVHO. At FVHO we do not discriminate against patients for any reason including but not limited to: gender, age, socio-economic status, ethnicity, religion or insurance status.
Patients have the right to speak confidentially with their healthcare provider and to have their privacy protected.
FVHO expects patients to assume responsibilities in their treatment outcomes. Patients are expected to comply with agreed upon treatment plans, communicate openly with the providers, and inform us about other treatments or medications you have had or are currently receiving.
The patient’s insurance is a contract between the patient and their insurance company. FVHO will file claims and follow-up with all third- party payers such as Medicare and private insurance carriers. Insurance and health information is often confusing, we encourage you to ask or seek help with any questions or concerns. If you do not have health insurance, FVHO will help you navigate the financial costs of treatment.
