FVHO and Heartland Hospice Care host advance directives seminar

On Thursday, April 20, 2017, Fox Valley Hematology & Oncology (FVHO), in partnership with Heartland Hospice Care, will host a workshop from 10 a.m. to 2 p.m., where the public can come at their convenience and receive assistance in completing health care power of attorney and medical living wills.

Power of attorney gives another person the authority to act on your behalf in legal or health matters, and a medical living will is written, legal instructions regarding your preferences for medical care if you are unable to make those decisions. These forms typically take 20 to 30 minutes to complete.

The workshop will take place at FVHO’s Health & Wellness Center, located at 3925 N. Gateway Drive, Appleton, in support of National Healthcare Decisions Day. FVHO physician Dr. Jack Swanson will be available during that time frame to assist and answer questions.
